Westside Support Service provides flexible floating support to people with a physical or sensory disability or long term health condition living in the London Borough of Hammersmith and Fulham or in the London Borough of Hounslow. To find out more about this service please read this leaflet.
We help people to understand what they need to do to sustain a tenancy.
People with a disability or long-term health condition are encouraged to develop essential skills to maintain a home, budgeting and money management advice, access benefits, aids and adaptations to live as independently as possible.
We encourage activities to help people remain as healthy as possible and to be inclusive financially and socially. This helps them build and maintain self esteem and personal well being. Every last Tuesday of the month Westside offer workshop and activites from external guests.
Support is delivered through regular individual sessions with a dedicated support worker. Clients have an agreed support programme to help them achieve personal goals and aspirations.
People with a disability or long-term health condition can refer themselves to Westside or can be referred by an advocate by completing this form. The completed form can be sent to the email address below or posted to:
